To tattoo at the event, you must register with the Louisville Metro Department of Public Health (LMDoPH). Follow these specific steps to ensure your application is processed correctly.

Step 1: Access the Application

Step 2: Fill Out the Form

The application is divided into several sections. Use the exact details provided below:

   

Page 1: Basics

Enter your Legal Name and Date of Birth.

Registration Type

Select Tattoo Artist (if not already registered in Louisville as an Artist) AND everyone must select Temporary Studio (workstation fee).

Page 2: Personal

Enter your SSN, phone, email, and home address.

Temporary Studio?

Select YES for “Is This a Temporary Tattoo Studio?”

Step 3: Event & Employer Details

When prompted for event and employer information on Pages 2 and 3, use these exact credentials:

  • Event Name: Tattoo Fest Louisville
  • Name of Temporary Studio: YOUR NAME AS LISTED ON ID
  • Start Date: 03/27/2026
  • End Date: 03/29/2026
  • Event Address: 221 S 4th St, Louisville, KY 40202
  • Employer’s Name: TATTOO FEST LOUISVILLE
  • Employer’s Phone: 260-302-2356
  • Employer’s Address: 221 S 4th St, Louisville, KY 40202

Step 4: Payment & Documentation

Pay Your Invoice: Follow the prompts on Page 4 to complete your payment.
Submit BBP via one of these methods:

  • Print a copy of your BBP to bring to the event.

Note: If your BBP is out of date or expiring before the event, we recommend this one based on ease and pricing. CLICK HERE.
Piercing is not allowed at this event. Please ensure you only select “Tattoo Artist” to avoid delays in your application.
